Preparing for Your NDIS Plan Review

A practical checklist to help you reflect on progress, refine your goals and advocate for the supports that matter most.

Reflect on your current plan

  • What goals have I achieved?
  • What progress have I made?
  • Which supports have worked well?
  • Which supports have not worked well?
  • What challenges am I still experiencing?

Review your goals

  • Are my current goals still relevant?
  • Have my circumstances changed?
  • What new goals do I have?
  • What would I like to achieve over the next 12 months?

Gather supporting evidence

  • Allied health and functional assessments
  • Support Coordinator or Recovery Coach reports
  • Psychology and Occupational Therapy reports
  • Progress notes and letters from providers

Identify gaps in your supports

  • What supports do I need that I do not have?
  • Are waiting lists affecting my access?
  • Have my support needs increased?

Building a Wellbeing Routine That Sticks

Practical strategies drawn from CBT and Behavioural Activation to support motivation, structure and daily wellbeing.

Action often comes before motivation.

Small, consistent actions often create positive change before motivation appears. A routine does not need to be perfect — it needs to be realistic and sustainable.

The three building blocks of wellbeing

Achievement

Activities that help you feel productive — household tasks, study, volunteering, applying for jobs, learning a new skill.

Connection

Activities that help you feel connected — talking with a friend, attending a community group, hobbies, support groups.

Enjoyment

Activities that bring pleasure or relaxation — music, gardening, reading, a favourite movie, time outdoors.

The 5 minute rule

If a task feels overwhelming, commit to just five minutes — walking, cleaning, job searching or journaling. Getting started is often the hardest part.

Progress, not perfection

Building new habits takes time. Celebrate small wins. You do not need to do everything — you only need to take the next step.
